Services
At Jordan OT, I provide mobile occupational therapy services for children aged 3-18, supporting them in developing essential life skills. I service Sunbury and surrounding areas. I work collaboratively with families to create an evidenced-based therapy plan tailored to your child's specific needs, ensuring support that is both meaningful and practical.

Process

Booking
Click the ‘Book Appointment’ button to open the intake form. Once you’ve submitted it, I’ll get in touch as soon as possible to arrange an initial phone call to discuss the next steps.

Initial Assessment
We’ll schedule a phone/telehealth session (about 45–60 minutes) to discuss your child’s strengths, needs, and goals. This session helps me understand your family’s priorities and plan how to best support you.
If you choose to continue, you’ll receive:

A Service Agreement to sign

A Service Plan — a clear roadmap outlining the goals we’ll work on together and the steps to achieve them

Therapy
Your child’s therapy program begins.
If you’re looking for ongoing sessions, we’ll organise the session dates and frequency together. Each session includes individualised activities, practical strategies and recommendations, and session notes/documentation provided after every visit.

If you’d like to discuss your child’s needs before booking, I’m more than happy to answer any questions. You can call, text, or email, and I’ll respond as soon as I can.
Email is usually best, as I may be in the middle of a session and unable to answer calls straight away. If you do call and I’m unable to pick up, please leave a voicemail and I’ll get back to you as soon as possible.
